Normothermic Regional Perfusion

Activity description

This activity consists of a recorded presentation from the "Essentials of Oncology, Solid Organ and Blood/Marrow Transplant Management for the Health Care Team" conference held in Scottsdale, Ariz., March 14–15, 2022.

Normothermic regional perfusion (NRP) plays a large role in improving graft survival and posttransplant outcomes through restoring continuous perfusion and reversing warm ischemic injury to organs donated after circulatory death. The utility of NRP has continued to expand, and data describing the use of NRP-heart-transplantation protocols such as cardiopulmonary bypass, thoraco-abdominal, or modified extracorporeal membrane oxygenation (ECMO) circuit have demonstrated effective and successful outcomes. This presentation will discuss the opportunities presented by NRP to increase the availability of donor organs and allow for extended patient survival time until transplant. The principles of NRP, effects on graft survival, potential complications, and ethical and legal concerns will be reviewed.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Review the status of NRP utilization in donation after circulatory death (DCD) organ transplantation, including principles, techniques, and effects on graft quality and clinical outcomes.
  • Discuss current data regarding heart-transplantation techniques using NRP/DCD.
  • Identify possible complications, as well as ethical and legal concerns, associated with NRP/DCD.
  • Describe the potential ability of NRP to increase the availability of donor organs for transplantation.

About the presenter

Dr. Victor Pretorius is a board-certified cardiothoracic surgeon who specializes in lead management. Pacemakers and implantable cardioverter-defibrillators (ICDs) deliver energy to the heart through thin, flexible wires called leads. When these leads are not working properly — because of infection, damage or surrounding scar tissue — they need to be removed. Dr. Pretorius performs a minimally invasive procedure called laser lead extraction to cut through scar tissue surrounding the lead, allowing it to be easily removed. Dr. Pretorius is surgical director of UC San Diego Health’s Heart Transplant Program, which provides comprehensive and compassionate care from a multidisciplinary team of experts in the fields of advanced heart failure, cardiac surgery and cardiac transplantation. As an associate professor in the Department of Surgery at UC San Diego School of Medicine, Dr. Pretorius publishes papers about the procedure and presents his findings at conferences around the world. He completed a fellowship in pulmonary thromboendarterectomy at UC San Diego School of Medicine and a residency in cardiac surgery at University of Alberta, Edmonton, Canada. He earned his medical degree (MBChB) from the University of Pretoria School of Medicine in his native South Africa. Dr. Pretorius is a Fellow of the Royal College of Surgeons of Canada (FRCSC). FRCSC is equivalent to board certification in the U.S.
