COVID-19 Vaccination Considerations Along the Transplant Continuum

This activity consists of a recorded presentation from the "Essentials of Oncology, Solid Organ and Blood/Marrow Transplant Management for the Health Care Team" conference held in Scottsdale, Ariz., March 14–15, 2022.

Many questions exist surrounding COVID-19 infection and vaccination protocols for the solid organ transplant community. While no cases of organ-transplant-transmitted infection have yet been described, this issue, as well as those pertaining to risks, testing, and vaccination in the pre- and posttransplant periods, are areas of concern. This session will review the current recommendations for screening and vaccination in both organ donors and organ recipients and will examine the current rationale and value of measuring COVID-19 antibody titers in light of possible low posttransplant sero-response and humoral response rates. Importantly, the current state of, and future implications for, transplantation in nonvaccinated patients will be discussed.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Describe the rationale, considerations, and current recommendations for COVID-19 screening, vaccination, and postvaccination testing during the pre- and posttransplant periods, as well as trends in immune responses to vaccination.
  • Discuss the potential for donor-derived COVID-19 infection and posttransplantation infection risks.
  • Review the current status and future implications for nonvaccinated patients in need of organ transplantation.
  • Describe the posttransplant management of COVID-19 infection, including what is known about the effects of immunosuppression on disease severity.

About the presenter

Dr. Cameron R. Wolfe is an associate professor of medicine, infectious disease. He has clinical interests in the management of immunocompromised patients, including HIV, solid organ and hematologic transplantation. Within that sphere, he specifically focuses on the outcomes of viral infections, including influenza, COVID, viral hepatitis and HIV. He has a particular interest in donor-derived infections. Dr. Wolfe serves as co-chair of the Duke University Coronavirus Task Force, overseeing all management decisions for COVID-19 treatment and detection on the university campus and in the health system. He has been involved with multiple studies of novel therapeutic agents for COVID-19, as well as having served on multiple safety review panels for treatments and vaccines. He is the chair of the ACC College Athletics COVID Advisory Panel, and NCAA Sports COVID Advisory Panel.
