Complex Medical Conditions in Children: Utilizing Personalized Treatments and Addressing Health Disparities
Activity description
Social determinants of health (SDoH), which include such factors as health care access, education level, social environment, and income, play a strong role in individuals’ physical and mental well-being, influence the development of chronic disease states and impact both short- and long-term health outcomes. Children with rare and complex medical conditions (CMC) and poorer SDoH face fundamental challenges and disparities in health, including decreased access to quality health care (often leading to delayed diagnoses and treatment), inadequate access to culturally appropriate care and a lack of suitable social and support services. This activity will focus on the connection between SDoH and how they may impact access to care, treatment and health outcomes for children with rare and CMC. Learners will be provided with strategies and resources that can address these negative effects. The importance of access to personalized care that they require to live their healthiest lives will be addressed.

Target Audience
This activity is designed to meet the educational needs of case managers, counselors, nurses, nurse practitioners, pharmacists, pharmacy technicians, physicians, PAs, psychologists, social workers, therapists and other health care professionals who care for the pediatric population.
Learning Objectives
At the end of this educational activity, participants should be able to:
- Describe the impact SDoH have on the diagnosis, treatment and outcomes of children with CMC.
- Address the importance of access to personalized care that children with rare and CMC require to live their healthiest lives.
- Identify practices and strategies that the interprofessional health care team may employ to address negative SDoH.
- State resources available to champion equitable change for children with CMC.

About the presenter
Dr. Dennis Z. Kuo received his medical degree from the University of Pennsylvania and completed his pediatrics residency at the University of North Carolina Hospitals. Dr. Kuo spent several years as a physician in a general pediatrics practice before completing a general academic pediatrics fellowship at Johns Hopkins University, where he received a Master of Health Science at the Johns Hopkins Bloomberg School of Public Health. His clinical and research interests include children with complex medical needs, the patient-centered medical home and health care system transformation.

The certificate will give the total CMEs possible for the offering. Physicians are advised to claim only the actual hours that they were present during the education. Social workers
As a Jointly Accredited Organization, OptumHealth Education is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. OptumHealth Education maintains responsibility for this course. Social workers completing this course receive 1.00 live continuing education credits.
Counselors and/or marriage and family therapists
CA: The Board of Behavioral Sciences has deferred CE course approvals to APA and ASWB for its licensees. See those approvals under Psychologists and Social Workers.
Other States: If your state is not specifically listed, nearly all state Counselor and MFT boards accept either APA or ASWB approval, or are reciprocal with other state licensing board approvals, such as those listed below. Check with your board to be sure.
