Skilled Nursing Facilities Series: Managing Multiple Complex Comorbidities

Activity description

This activity focuses on the complexities of managing patients in long-term care settings who present with multiple competing comorbidities. Participants will examine key take-home points for each major condition and reinforce the application of guideline-directed medical therapy (GDMT) tailored to the unique challenges of this population. They will review how to evaluate and manage individuals where treatment decisions require balancing competing priorities and individualized care plans. Emphasis on the critical role of prognostication and advanced care planning in guiding clinical decisions will also be discussed. By integrating prognostic tools and patient-centered discussions, participants will learn strategies to align medical management with patient goals, improve quality of life and support informed decision-making. This activity equips health care professionals (HCPs) with practical skills to deliver evidence-based, compassionate care in complex long-term care scenarios.

This activity is designed to meet the educational needs of nurse practitioners, nurses, PAs, case managers, physicians, psychologists, social workers and other HCPs who care for older adults.

At the end of this activity, participants should be able to:

  • Outline key take-home points for each topic and specific comorbidities.
  • Describe GDMT and address the unique challenges and complexities of managing patients in long-term care settings. 
  • Discuss strategies to evaluate and manage patients with multiple competing complexities in long-term care settings.
