Transition 101: The Importance of Transition Planning and Approach for Pediatric to Adult Healthcare

This activity is designed to meet the educational needs of nurses and other health care professionals (HCPs) who are interested in learning about transition planning.
At the end of this educational activity, participants should be able to:
- Recognize the impact of disabilities in healthcare and within the community.
- Identify the importance of timely planning for a transition from Pediatric to an Adult model of care to include none healthcare considerations.
- Discuss how to access valuable transitional resources and support for individuals transitioning to an Adult model of care.

In accordance with the Accreditation Council for Continuing Medical Education's (ACCME) Standards for Integrity and Independence in Accredited Continuing Education, Optum Health Education (OHE) requires all those in control of educational content to disclose their financial relationships with ineligible companies within the prior 24 months. To apply for credit types not listed above, participants should use the procedure established by the specific organization from which they wish to obtain credit.
