Managing Members With Chronic Kidney Disease

Chronic kidney disease (CKD) is a health condition where the kidneys lose normal function over time; this causes poor filtering of the blood. The main causes of CKD are uncontrolled hypertension and diabetes. When CKD is diagnosed, lab tests are used to determine its stage. As a case manager, it is not uncommon to manage individuals who have multiple comorbidities, including CKD. This activity will provide an overview of CKD; how to complete a phone assessment for complications; and how to discuss medications, interventions and support measures.

Social workers
As a Jointly Accredited Organization, OptumHealth Education is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. The Commission for Case Manager Certification has approved this program for a maximum of 1.00 clock hours for Certified Case Managers (CCM).
Attendance
A certificate of attendance will be provided to learners upon completion of activity requirements, enabling participants to register with licensing boards or associations that have not been pre-approved for credits.
