Activity description
Behavioral Health Grand Rounds is a monthly, interdisciplinary forum focused on advancing evidence‑based, patient‑centered care for complex behavioral health conditions. Through case‑based discussion, clinicians examine social drivers of health, gaps in care and practical strategies to improve outcomes across diverse patient populations. Sessions rotate across key focus areas to keep health care professionals (HCPs) current on evolving best practices and collaborative approaches to care.
Learning objectives
At the end of this educational activity, participants should be able to:
- Identify gaps in care and available resources based on diagnosis, age group and level of care and determine when to request facility or system‑level assistance.
- Recognize social drivers of health and explain how they contribute to readmissions and hinder an individual’s ability to thrive in the community.
- Identify evidence‑based interventions and best practices—including therapeutic strategies, counseling approaches and support services—that improve health care outcomes.
- Discuss preventive strategies that increase awareness of risk factors and enhance communication within the health care team, as well as with families, caregivers and community partners.
- Apply session learnings to clinical practice, care coordination and utilization review processes to support improved patient outcomes.
