Activity description
Behavioral Health Grand Rounds is a monthly, interdisciplinary educational forum designed to enhance evidence‑based, patient‑centered care for complex behavioral health conditions encountered across clinical settings. The series provides a structured, collaborative environment where health care professionals (HCPs) engage in case‑based learning to address diagnostic, therapeutic and systems‑level challenges affecting patient outcomes.
Through interactive discussion of real‑world cases, participants examine social drivers of health, identify gaps in care and resource utilization and explore evidence‑based and emerging interventions across the continuum of behavioral health services. Emphasis is placed on practical strategies that support coordinated, team‑based care and improve clinical decision‑making in diverse patient populations.
This activity is designed to meet the educational needs of counselors, nurses, nurse practitioners, physicians, physician assistants, psychologists, social workers, and the health care team involved in the assessment and management of behavioral health conditions impacting individuals and communities.
Learning objectives
At the end of this educational activity, participants should be able to:
- Identify gaps in care and available resources based on diagnosis, age group, and level of care and determine when to request facility or system‑level assistance.
- Recognize social drivers of health and explain how they contribute to readmissions and hinder an individual’s ability to thrive in the community.
- Identify evidence‑based interventions and best practices—including therapeutic strategies, counseling approaches and support services—that improve health care outcomes.
- Discuss preventive strategies that increase awareness of risk factors and enhance communication within the health care team, as well as with families, caregivers and community partners.
- Apply session learnings to clinical practice, care coordination and utilization review processes to support improved patient outcomes.
