Activity description

Effective, efficient health care is dependent upon value-based care delivery that aligns with the Institute for Healthcare Improvement (IHI) quadruple aim: enhancing the patient care experience, improving population health, reducing health care costs and care team well-being and health equity. This conference will provide participants with innovative strategies designed to improve coordinated care delivery and achieve the IHI quadruple aim. Care systems will share innovative methods for alternative care delivery including incorporating community health workers into a care system, using data to drive results within medication therapy management programs and innovative behavioral health programs that reimagine the behavioral health experience for patients. Additionally, engaging sessions with leaders in value-based care delivery will share strategies and viewpoints on how to gain physician engagement within this model, as well as share insights into the future of value-based care.

Target Audience

This activity is designed to meet the educational needs of case managers, nurses, nurse practitioners, PAs, pharmacists, pharmacy technicians, physicians and social workers.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • Describe innovations in value-based care delivery as discussed by a panel of care system partners, including actionable ideas on how to create change within organizations to achieve the IHI quadruple aim.
  • Identify strategies to achieve coordinated care through alternative care delivery models such as robust MTM programs and adding community health workers to your care team.
  • Discuss approaches to change the way behavioral health care is being provided to patients through emergency department services and integration within primary care.
  • Define key trends within marketing, specifically around the member experience, that will impact the experience members have when working with a health plan. .
