Series description

This series will offer insights for an interdisciplinary team of healthcare providers who deliver care for residents in nursing homes and in long-term care facilities. The goal is to improve interdisciplinary collaboration, communication, assessment skills and interventions that will aid in building provider, staff and patient confidence in the ability to evaluate and treat patients appropriately, and to help mitigate risk and enable development of a better care plan for optimal outcomes.

Continuing education 

This series is designed to meet the educational needs of nurses, nurse practitioners, social workers and other HCPs who have an interest in increasing their knowledge of educational topics related to care in Skilled Nursing Facilities (SNFs).
