Managing Transitions of Care: Best Practices for Reducing Hospital Readmissions

The rate of inpatient readmissions at Reliant Medical Group (RMG) has increased over the past few months. Recognizing that, nationally, a substantial proportion of 30-day readmissions are preventable. This activity will provide our primary care workforce with the latest research on readmission and discuss our current internal efforts to minimize this occurrence.

At the end of this educational activity, participants should be able to:
- Identify the factors that increase the chance of hospital readmission and how post-discharge visits with a primary care provider reduce the risk of readmission.
- Discuss LACE index scoring and how it is used at RMG to avoid unnecessary hospital readmissions.
- Describe the tools and resources that are available to providers and care teams to reduce the risk of readmission.
