Medical Home Care for the Child with Medical Complexity: Care Coordination and Shared Decision Making
Children with medical complexity (CMC) have multiple chronic conditions and require an array of health care professionals (HCPs) to manage their care. Effective care coordination (CC) is essential to addressing the fragmented and often inefficient care that these children frequently encounter, and building systems that work for their complex health care needs. This activity will define CC and discuss the impact of effective CC and shared decision-making for optimizing care of CMC in a medical home. The fundamentals, infrastructure, tools, leadership and outcomes of CC in the medical home for CMC will be presented.

This activity is designed to meet the educational needs of counselors, educators, nurses, nurse practitioners, pharmacists, pharmacy technicians, PAs, physicians, psychologists, social workers, therapists and other HCPs who have an interest in the care of children with medical complexity.
At the end of this educational activity, participants should be able to:
- Define care coordination (CC), care integration, care management, case management and care in a medical home.
- State the impact of effective CC for the optimal management of children with medical complexity, and complications that may arise when a team-based approach is not used.
- Recognize the impact of incorporating shared decision-making to fully optimize care in the medical home.
- State the fundamentals, infrastructure and outcomes of CC for children with medical complexity.
- List the tools, training and leadership required to implement CC.

About the presenter
Dr. Dennis Z. Kuo received his medical degree from the University of Pennsylvania and completed his pediatrics residency at the University of North Carolina Hospitals. Dr. Kuo spent several years as a physician in a general pediatrics practice before completing a general academic pediatrics fellowship at Johns Hopkins University, where he received a Master of Health Science at the Johns Hopkins Bloomberg School of Public Health. His clinical and research interests include children with medical complexity, the patient-centered medical home, and health care system transformation.
