Optimizing Care for Children with Medical Complexity: The Team-Based Approach

This activity consists of recorded presentations from the 28th Annual National Conference held in Minneapolis, Minn., on Oct. 15, 2019.

Children with special health care needs (CSHCN) have, or are at increased risk for chronic physical, developmental, and behavioral or emotional conditions and require an array of HCPs to manage their care. An effective multidisciplinary team approach designed to work on the common goal of improved patient-centered care is essential to addressing the fragmented and often inefficient care that these children frequently encounter and building systems that work for their complex health care needs. This presentation will address the impact of interprofessional collaboration in creating a multidisciplinary team approach for CSHCN.

This activity is designed to meet the educational needs of case managers, nurses, pharmacists, pharmacy technicians, physicians, psychologists, social workers and other health care professionals who are interested in children with special health care needs.

Learning Objectives

At the end of this educational activity, participants should be able to:

  • State the key components of establishing an effective multidisciplinary team to optimally manage CSHCN.
  • Explain the importance of communication as a core component of an interprofessional team approach to managing CSHCN.
  • Compare and contrast effective communication methodologies among members of the interprofessional health care team and identify practical solutions for effective collaboration for CSHCN.
