Type 2 Diabetes Mellitus: Diagnosis, Management and Documentation
Activity description
This integrated learning activity provides a holistic overview of type 2 diabetes mellitus (T2DM), focusing on screening and diagnosis, clinical management and documentation of complications. Learners will explore evidence-based practices and standards from the American Diabetes Association (ADA) to enhance clinical decision-making and patient outcomes. The importance of early detection of T2DM through risk-based screening and standardized diagnostic criteria will be emphasized along with a discussion on the management component that will focus on maintaining glycemic control, preventing complications and optimizing treatment plans through lifestyle interventions and medication. Reinforcing best practices for accurate coding and clinical documentation, especially in cases involving complications, will be highlighted. Learners will gain skills to ensure clarity, specificity and continuity of care in medical records, even when T2D is well controlled.

Module 1: Screening and diagnosis
At the end of this educational activity, participants should be able to:
- Define T2DM.
- Review screening guidelines and diagnostic criteria for T2DM
Module 2: Management
At the end of this educational activity, participants should be able to:
- Recall recommendations for assessing glycemic status and glycemic goals for patients with T2DM
- Review the key aspects of the comprehensive medical evaluation for patients with T2DM
- Discuss evidence-based practices for both nonpharmacologic and pharmacologic management of T2DM.
Module 3: Documenting and coding for complications
At the end of this educational activity, participants should be able to:
- Identify key documentation principles for T2DM with complications
- Apply a generalized coding framework to complications of T2DM.
- Accurately document and code complications of T2DM to the highest level of specificity.
