Virtual Case Study: Current versus History of Cancer

Documenting and coding active cancer vs a history of cancer in a patient’s medical record can be difficult and at times confusing. Medical treatment and patient presentations often dictate the appropriate cancer codes to document in the medical record. This activity reviews two patient presentations that will assist HCPs in making appropriate documentation choices in reference to active cancer and a history of cancer diagnosis.

Learning objectives
At the end of this educational activity, participants should be able to:

  • Recognize patients who require specific documentation for current cancer and a history of cancer.
  • Correctly code the patients’ cancer diagnosis based on clinical data.
  • Document patient records to the highest level of required specificity.
