Heart Failure: AgeWise

Activity description

These 4 educational modules will provide a structured, evidence-based approach to improving the assessment, diagnosis and management of heart failure across its continuum. Learners will explore the critical role of early assessment and the use of validated tools and current guidelines to identify heart failure promptly. The modules will emphasize the application of consistent criteria for echocardiogram referral and highlight the clinical value of echocardiography as the gold standard for diagnosis. Participants will learn heart failure stages and classifications, applying management strategies that span multiple stages to slow disease progression and optimize symptom control. The final module focuses on guideline-directed medical therapy (GDMT), including rapid initiation and titration for heart failure with reduced ejection fraction (HFrEF), evidence-based treatment strategies for heart failure with preservation ejection fraction (HFpEF) and the importance of continued therapy for individuals with heart failure with improved ejection fraction (HFimpEF). Through this integrated curriculum, participants will gain practical skills and clinical insights to deliver high-quality, guideline-aligned care for individuals with heart failure.

This activity is designed to meet the educational needs of nurses, physicians and other health care professionals.

Module 1: Clinical importance and assessment
At the end of this educational activity, participants should be able to:

  • Recognize the importance of early assessment for heart failure.
  • Identify evidence-based tools and current guidelines for heart failure assessment.

Module 2: Diagnosis
At the end of this educational activity, participants should be able to:

  • Apply consistent criteria for echocardiogram referral.
  • Explain the clinical value of echocardiography as a gold standard in heart failure diagnosis

Module 3: Management
At the end of this educational activity, participants should be able to:

  • Recall heart failure stages and classifications.
  • Explain management strategies that apply across multiple heart failure stages.
  • Apply interventions to slow or prevent disease progression and manage symptoms.

Module 4: Guideline-directed medical therapy
At the end of this educational activity, participants should be able to:

  • Review GDMT for HFrEF and key principles for rapid initiation and titration of medications.
  • Recognize and implement evidence-based treatment strategies for HFpEF.
  • Recall the importance of continued treatment in patients with HFimpEF.
