Heart failure is a common chronic condition associated with high morbidity and mortality. In this activity, we discuss specific diagnoses related to heart failure and how to document cases to the highest degree of specificity. We also discuss a related condition called secondary hyperaldosteronism, explain the relationship between the two and how to diagnose and document the condition.
