Palliative Care in Primary Care

These materials are only intended for those that attended the live activity held on October 10, 2023.
Palliative care within the primary care setting facilitates a proactive approach to the early identification of the needs of patients with serious illness and allows for interventions addressing their needs for palliative services. This activity will provide participants with an overview of the role of palliative care services in the primary care setting across both Reliant and Atrius Health. It will also review indications for palliative care referral and describe an optimal approach to discussing advance care planning with patients and their families.

Target Audience
This activity is designed to meet the educational needs of nurse practitioners, PAs and physicians who have an interest in learning how to better integrate palliative care services into the primary care setting.
Learning Objectives
At the end of this educational activity, participants should be able to:
- Articulate the definition of palliative care and identify its key components.
- Evaluate and identify appropriate indications for referral to palliative care services.
- Describe the role of advance care planning discussions with patients and their families and optimal approaches to conducting these conversations.

The American Academy of Nurse Practitioners Certification Program (AANPCP) accepts credit from organizations accredited by the ACCME and ANCC.
Attendance
A certificate of attendance will be provided to learners upon completion of activity requirements, enabling participants to register with licensing boards or associations that have not been pre-approved for credits. To apply for credit types not listed above, participants should use the procedure established by the specific organization with which they wish to obtain credit.
