- Completing the Pre- and Post-Activity Assessments, Activity Evaluation, and Application for Certificate of Credit forms

- Within two weeks of completing the post-activity assessment and evaluation, you will receive your Certificate(s) of Social Worker Credit from moreinfo@optumhealtheducation.com.
