Threading the Needle: Practical Considerations in the Modern, Evidence-based Management of Type 2 Diabetes
Activity description
The development of type 2 diabetes (T2D), a disease that affects more than 30 million individuals in the United States, is influenced by a number of risk factors, genetic predisposition and multiple health behaviors. The focus of medical care for individuals with T2D includes formulating an individualized treatment plan that balances control of blood sugar levels, reduces risks for cardiovascular and microvascular complications and optimizes medication regimens. Additionally, health care practitioners (HCPs) are in a position to assist patients with T2D in developing meaningful health behavior changes and setting healthy lifestyle goals to maximize health outcomes and slow disease progression. This activity will assist learners in identifying optimal lifestyle and behavioral changes that can impact disease development and progression in their patients and will discuss practical strategies and forms of support that HCPs can offer to assist patients in setting healthy lifestyle goals. Learners will examine how appropriate prescribing decisions must be based on a comparative analysis of diabetes medications and will also review disease etiology and biological changes seen during disease progression. Lastly, the presenter will discuss policy and system changes that can best assist in integrating evidence into daily practice for the purpose of maximizing health in individuals with T2D.

Target Audience
This activity is designed to meet the educational needs of case managers, counselors, educators, nurses, nurse practitioners, pharmacists, pharmacy technicians, PAs, physicians, psychologists, social workers, therapists and other HCPs who have an interest in the care of patients with T2D.
Learning Objectives
At the end of this educational activity, participants should be able to:
- Describe the causes of T2D and the biological changes that occur during the development and progression of the disease
- Identify lifestyle and behavioral changes that impact disease development and progression
- Discuss practical strategies and forms of support that can assist patients in making meaningful behavioral changes and setting healthy lifestyle goals
- Recognize how the comparative actions, benefits, harms and costs of different diabetes medications must factor into appropriate prescribing decisions
- Describe at least two policy or system changes that could help integrate evidence for maximizing health in people with T2D into daily practice

About the presenter
Dr. Ronald Ackermann, MD, MPH, is Professor of Medicine and Medical Social Sciences at Northwestern University Feinberg School of Medicine in Chicago, Illinois. He directs Northwestern University’s Institute for Public Health and Medicine and is Co-Director for its Center for Diabetes and Metabolism. He is a clinical and population health researcher and practicing primary care physician with experience in the development, implementation and evaluation of multi-sector programs and policy interventions to improve population health and health equity for adults with T2D and prediabetes. Dr. Ackermann received his medical degree from the University of Wisconsin and his Master of Public Health degree from the University of Washington. He completed his internship and residency in internal medicine at the University of Texas Health Science Center, San Antonio, and his postdoctoral fellowship at the University of Washington, Health Services Research. Dr. Ackermann was a Clinical Scholar/VA Scholar and Physician Faculty Scholar of the Robert Wood Johnson Foundation.
