Living with a rare disease impacts individuals and their family/caregivers, both physically and mentally. This podcast will share insights and perspectives directly from Erica Barnes and provide strategies that the interprofessional health care team can utilize to better support those impacted by rare diseases such as leukodystrophies.

This activity will provide an overview of geriatric and frailty assessment tools that can assist in guiding cancer- and transplant-related decision making through the risk stratification of older individuals before therapy is initiated. Data surrounding the relationship among increased age, geriatric- and frailty-assessment results, and disease outcomes will also be explored. Key considerations in how to best incorporate these sometimes time-intensive assessments into practice will be outlined, along with discussion of how interprofessional collaboration among members of the health care team in the creation of care plans can facilitate quality care.

In this podcast Nealie White, an individual with Pomp disease, will share her insights and perspectives, and panelists will provide strategies that the interprofessional health care team can utilize to better support people who are impacted by rare diseases.

This activity will discuss The Strategies to Reduce Injuries and Develop Confidence in Elders (STRIDE) trial, which aims to evaluate the effectiveness of evidence-based strategies to reduce serious fall-related injuries,. In addition, faculty will identify risk factors that contribute to falls in the older adult as well as for individuals in SNFs, and provide interventions to reduce the risk of falls. The importance of a multidisciplinary approach in fall management will also be explored.
