Activity description

SAFE: Speak. Act. Fix. Evolve. is a structured framework designed to guide teams through fair, consistent and transparent responses to unwanted events. This activity empowers participants to uphold organizational values, foster psychological safety and drive continuous improvement. Through interactive scenarios, decision tree applications and collaborative exercises, participants explore how to speak up about concerns, take informed action, fix contributing issues and improve systems and behaviors.
This activity is designed to meet the educational needs of nurse practitioners, nurses, pharmacists, pharmacy technicians, PAs, physicians, psychologists, social workers and other health care professionals who have an interest in learning about SAFE.
At the end of this educational activity, participants should be able to:
- Explain the purpose and structure of the SAFE framework and its principles.
- Apply the SAFE decision tree to evaluate behaviors and system factors contributing to unwanted events.
- Distinguish between outcome-related and behavior-related events using the “duty to produce outcomes” branch.
- Demonstrate how consistent and compassionate responses build trust and support team accountability.
- Collaborate with team members on root cause analysis and system improvement efforts to strengthen reliability and resilience.
- Identify opportunities for continuous learning and adaptation following event resolution.

In accordance with the Accreditation Council for Continuing Medical Education's (ACCME) Standards for Integrity and Independence in Accredited Continuing Education, Optum Health Education (OHE) requires all those in control of educational content to disclose their financial relationships with ineligible companies within the prior 24 months. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Optum Health Education maintains responsibility for this course. Social workers completing this course receive 2.50 enduring continuing education credits.
Attendance
A certificate of attendance will be provided to learners upon completion of activity requirements, enabling participants to register with licensing boards or associations that have not been pre-approved for credits. To apply for credit types not listed above, participants should use the procedure established by the specific organization with which they wish to obtain credit.
