About This Conference

There is strong evidence that access to well-coordinated and integrated health care will reduce complications associated with ongoing chronic diseases. According to the Centers for Disease Control and Prevention (CDC), chronic diseases are the leading cause of death and disability in the United States. The CDC identifies the four common causes of chronic disease as lack of physical activity, poor nutrition, tobacco use and excessive alcohol consumption. How can medical professionals impact these behaviors? Gain insight at this conference from our expert faculty.

“Innovations in Wellness and Disease Management” will relate methods and mediums used to promote effective, personalized health care and wellness strategies designed to improve the quality of life for those living with chronic medical conditions.
