Documenting cancer can often cause frustration for health care professionals (HCPs). Understanding the difference between when a cancer should be documented as “active” vs. “history of” is critical. Making cancer documentation as specific and accurate as possible can help HCPs avoid pitfalls, decrease frustration and minimize insufficiencies.
