Activity description

This conference is dedicated to exploring the latest innovations and evidence-based research that drive excellence in patient care, enhance outcomes and optimize benefit management. Attendees will have the unique opportunity to acquire expert knowledge from leading voices in health care on the most pressing challenges in population health management.

It has been designed to meet the educational needs of professionals managing large patient populations and/or their benefits, including: clinical executives/leaders, advanced practice and front-line clinicians, medical directors, nurses, physicians, pharmacists, psychologists and social workers.

Learning objectives

As a result of attending this conference, learners should be able to:

  • Incorporate the latest 2025 Global Initiative for Chronic Obstructive Lung Disease (GOLD) recommendations into clinical practice to improve early detection, diagnostic accuracy, and personalized treatment of chronic obstructive pulmonary disease (COPD).
  • Review the science, clinical applications and ethical considerations of gene therapy, with a focus on its transformative potential in treating genetic disorders such as sickle cell disease.
  • Evaluate evidence-based management strategies and tools to detect heart failure in its early stages to reduce hospitalizations and improve quality of life.
  • Describe how to recognize, diagnose and manage non-alcoholic fatty liver disease (NAFLD), now recognized as metabolic dysfunction-associated steatotic liver disease (MASLD), through evidence-based strategies to improve patient outcomes and reduce progression to advanced liver disease.
  • Assess the bidirectional relationship between mental and physical health and promote integrated care models that improve outcomes for patients with complex needs.
  • Identify the mechanisms, indications, and clinical benefits of GLP-1 agonists and integrate them effectively into care plans for patients with obesity and type 2 diabetes.
  • Discuss how to enhance the early diagnosis of dementia, initiate patient-centered conversations, and implement comprehensive care strategies to support patients and families.

Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity.
