The process of advanced care planning (ACP) is widely recognized as a method to support patient self-determination, facilitate decision-making and promote better care. The responsibilities of the clinician are to utilize diagnostic tests, observe vital signs, diagnose illness, administer treatment, provide a prognosis based on treatment progress and document the patient's medical history. ACP will not succeed without effective communication between clinicians and patients, as well as family members and caregivers as appropriate. Existing barriers when engaging patients in ACP may be challenging for health care professionals. This activity will describe the role of the clinician in ACP, identify existing barriers and discuss how to overcome those barriers.

It will discuss the challenges of problematic polypharmacy and identify strategies for recognizing deprescribing opportunities, as well as describe the role of governance strategies in deprescribing initiatives. Deprescribing, the process of tapering or stopping medications that may no longer be beneficial or may be causing harm, is crucial for improving patient outcomes and enhancing quality of life.
