We are pleased to include you as a presenter at Optum Health Education’s 32nd Annual National Conference, to be held October 2-4, 2023, at the Hyatt Regency Minneapolis.

Please review and complete the tasks below for your participation:

Complete and return the Faculty Disclosure Form as soon as possible.

Complete and return the Faculty Agreement and Honorarium Contract.
Finalize travel details by August 18, 2023.

Travel Arrangements | Due August 18, 2023:

Air Travel Information

We request airfare is booked at least 6 weeks prior to departure date. Flights may be booked by Optum Health Education's travel coordinator. If you prefer to book airfare on your own for reimbursement you are free to do so. Upon booking, please email a copy of your itinerary to Bethany.severson@optumhealtheducation.com to use as a receipt. Standard coach class is reimbursable.

Hotel Reservation Request

Optum Health Education will arrange presenter hotel accommodations. One to two nights will be covered by our department. If you wish to extend your stay we can make the reservation accordingly and the additional nights will be charged to you.
