Residents in skilled nursing facilities typically have multiple medical diagnoses requiring ongoing management. A key to resident health is for nursing staff to be familiar with individual resident norms so they can identify potential problems and facilitate the necessary provider notification, which may impact outcomes and avoid unnecessary hospitalizations. Because of the risks of hospitalization in frail, elderly residents, clinicians/providers should be aware of these risks when weighing the decision to send a resident to the hospital versus treating them in the facility. This activity will identify conditions that require hospitalization and the potential risks and complications that hospitalizations can pose. The importance of timely identification of resident problems and accurate communication with health care providers who deliver care for residents will also be discussed.

This on-demand webcast addresses the current gap between ideal practices (based on current knowledge) and actual practices in how medical practitioners manage patients with behavioral health disorders. Proper diagnosis, timely referrals, and effective treatment plans can aid in bridging the practice gap and improve patient outcomes for those with behavioral health conditions. Learners will gain insight regarding the primary care physician’s (PCP’s) role in behavioral health, how to implement effective referrals, develop skills to implement post-discharge care with timely and effective treatment plans for patients transitioning from higher levels of care back to primary care, provide strategies to combat stigma surrounding behavioral health, and improve coordination of care between PCPs and behavioral health providers. This live webcast is designed to provide updated knowledge on the diagnosis and management of coronary artery disease (CAD) with a focus on the benefits of coronary computed tomography angiography (CCTA) and coronary artery calcium (CAC) scoring. It will explore the diagnostic accuracy, patient safety, and clinical outcomes associated with CCTA and compare this approach to traditional functional-stress-testing methods. The utilization of CAC scoring for risk assessment and in the evaluation of low-risk chest pain will be highlighted, and the role of the primary care physician (PCP) in implementing a CCTA-first approach will be discussed.
