The process of advanced care planning (ACP) is widely recognized as a method to support patient self-determination, facilitate decision-making and promote better care. The responsibilities of the clinician are to utilize diagnostic tests, observe vital signs, diagnose illness, administer treatment, provide a prognosis based on treatment progress and document the patient's medical history. ACP will not succeed without effective communication between clinicians and patients, as well as family members and caregivers as appropriate.
