Meagan Ausborn, RN
Director of Clinical Operations, UnitedHealthcare Community & State, Clinical Redesign

Meagan has over has over 17 years of healthcare experience serving people with complex medical, behavioral, and social needs.  Meagan is a Registered Nurse, Certified Yoga Teacher and Level 2 Reiki practitioner with a certification in Karmic Reiki.   Meagan’s diverse healthcare experience includes implementing and growing a Health coach program and complex care model for a critical access hospital and clinic system.  Meagan directed community, corporate and employee wellness and educational initiatives.  She previously served as a forensic sexual assault nurse examiner for Story County and a board member for Eastern Story County Board for Youth and Shelter Services, bringing a trauma-informed and patient-centered approach to vulnerable populations. Most recently, Meagan directed a 25 bed inpatient acute and skilled unit as well as overseeing ED and mobile MICU.   Meagan now leverages her myriad years of experience and passion for the underserved population to support and lead patient-facing staff to provide high-quality, trauma-informed and patient-centered care.  Meagan is based in Iowa, has three delightful young daughters and enjoys spending time with them, reading and yoga.

Nina Aniskevich, MSW
Behavioral Health Care Advocate, Optum Population Health Management, Medicaid

Nina Aniskevich is a Licensed Clinical Social Worker and Certified EMDR therapist. She has close to 15 years of experience in treating adults with mental health and substance use disorders. Nina is a Behavioral Health Care Advocate for Optum Population Health Management Medicaid Division.  Her focus is working with members who have been diagnosed with Opioid Use Disorder and are in need of assistance with warm handoffs, medication assisted treatment resources and behavioral health case management.   Nina also has a part time private practice where she provides clinical supervision and behavioral health therapy to individuals, families and groups focusing on trauma and recovery.  She is known for being non-judgmental and compassionate, which allows individuals to feel comfortable while working towards recovery.  One of Nina's main goals is to help individuals and families who are affected by trauma, as well as provide education to continue promoting change. Nina’s favorite season is summer, where she enjoys the beach and running outside.

Jeffrey Brenner, MD
SVP Clinical Redesign, UHC Community & State

Dr. Brenner is Senior Vice President of the Clinical Redesign team. Before joining UHC, Dr. Brenner served as a family physician working in Camden, NJ, where he owned and operated a solo-practice, urban family medicine office that provided a full-spectrum of family health services for the local Medicaid population. Recognizing the need for a new way for hospitals, providers and community residents to collaborate, he founded the Camden Coalition of Healthcare Providers in 2003, where he served as Executive Director until 2017. Dr. Brenner’s, and the Coalition's, innovative work was profiled in the 2011 New Yorker article “The Hot-spotters” and on PBS Frontline. In 2013 Dr. Brenner was honored with the MacArthur “Genius” Fellowship for his work, and in 2014 was elected to the Institute of Medicine. He holds a bachelor’s degree in Biology from Vassar College and graduated from Robert Wood Johnson Medical School. He completed his residency with Swedish Family Medicine in Seattle, WA.

Brad Chamberlain, LISW
Care Advocate, UnitedHealthcare Community & State, Clinical Redesign

Brad Chamberlain is a licensed independent social worker (LISW) professional with master’s level experience in public policy advocacy, non-profit administration, medical trauma social work and case management with vulnerable adult and youth populations. Brad has a rich background having served in a variety of positions with Lifepath Systems Collin County Texas, Children's Medical Center Dallas, The Arc of Dallas, and the Des Moines Veterans Administration Domiciliary Program. He previously served on the Texas Council of Governments - Children’s Special Interest Group and the AIDS Arms - Executive Planning Committee. Brad has a passion for working with individuals with complex trauma histories. Through Clinical Redesign's home-based care program, he provides therapy to adults with behavioral health needs in their homes utilizing a variety of treatment modalities.  He is certified in Cognitive Behavioral Therapy Supervision (CBT-S) and has utilized art therapy in working with PTSD veterans.

Meredith Clemmons, LCSW, CCM
Associate Director, Product Development, Optum

Meredith Clemmons is a licensed clinical social worker and certified case manager. She obtained her undergraduate and graduate degrees from the University of Southern Mississippi. She has worked in the behavioral health field for twenty years and has worked with geriatric, child and adolescent populations as a therapist on in-patient psychiatric units. Before coming to Optum in 2012, she worked as a manager for a social work care coordination department at an in-patient medical facility. It was there that she saw firsthand the connection between medical and behavioral health needs. Since joining Optum, Meredith has worked as a Field Care Advocate for Mississippi public sector, and as a clinical and medical operations manager within the Whole Person Care Model. She currently works as an Associate Director in Product Development, focusing on behavioral health projects in the government sector.

Amy Fynmore, CWDP
Employment & Vocational Administrator, United Healthcare Community & State, Clinical Redesign

Amy has a bachelor of arts in Sociology and is a Certified Workforce Development Professional. She currently helps support our complex care members via the Clinical Redesign Team’s Housing + Health program. Amy has previously worked for various community based organizations in addition to the City of Phoenix as a Career Advisor. Amy has also helped make significant pivots within our evolution of platforms of care focused on job readiness and a “purpose” program, which focuses on what brings joy to an individual’s life. Amy has exemplified the importance of change from traditional to compassionate-trauma informed care. During Amy’s tenure, Clinical Redesign Team’s complex care management programs have supported the transition of many complex individuals back into the community in part due to Amy’s innovative approach to care.

Kerianne Guth, MSW
Director, Strategic Business Initiatives, UnitedHealthcare Community & State, Clinical Redesign

Kerianne brings 10 years of experience working with vulnerable and underserved individuals and families and is excited to translate years of providing trauma-informed care and management into strategic business initiatives with the Clinical Redesign Team. She most recently served as the Administrative Director for Behavioral Health and Addiction Medicine at Cooper University Health Care in Camden, NJ with responsibility for the Department of Psychiatry and Division of Addiction Medicine. She was a founding team member of the Urban Health Institute at Cooper, a dedicated business unit transforming healthcare delivery. Kerianne led the start-up of the Division of Addiction Medicine including medication treatment for substance use disorders, behavioral health support, inpatient consultation, and trauma informed population health initiatives. She has previous experience as a social worker following her Master’s in Social Work degree from the University of Pennsylvania. Kerianne is based outside of Philadelphia with her husband, son and rescue dog. 

Jean Mash, LSCSW, LCAC
Clinical Program Director, UnitedHealthcare Community & State, Clinical Redesign

Jean is the Clinical Program Director within the Clinical Redesign team.  She has a master’s degree in Social Work and is a clinically licensed social worker (LSCSW) and addiction counselor (LCAC).  Jean came to UHC as a manager during the Kansas implementation in 2012 and has been part of the home-based care program since its inception. Prior to coming to UHC, she spent 15 years working in community mental health specializing in the diagnosis and assessment of mental health conditions in both the adult and juvenile populations.  During this time, Jean was a site leader for the Crisis Response Team (CRT) and was certified by the Crisis Prevention Institute (CPI) as a trainer for Nonviolent Crisis Intervention.  She is also an adjunct professor at Wichita State University where she teaches graduate level Diagnosis and Assessment for the School of Social Work.  

Nellie Nekouie, MSW
Arizona State Director, UnitedHealthcare Community & State, Clinical Redesign

As a Certified Peer Support Specialist, Nellie uses her insight into the experience of others to lead a team focused on providing quality trauma-informed services to individuals with complex needs. During her tenure with Clinical Redesign, Nellie’s passion and energy have enabled her to break down operational and systemic barriers, helping to ensure members are given a fair chance at life. Before joining Clinical Redesign, Nellie served in client management, network development and financial planning. Nellie also leads the Arizona Clinical Redesign Community Data Toolbox initiatives in collaboration with Arizona Medicaid, justice involved organizations, schools, and other community leaders. 

Rodrigo Olivares, MSW
CBO/VP Market Operations, UnitedHealthcare Community & State, Clinical Redesign

Rodrigo is Chief Behavioral Officer of the Clinical Redesign team, CRT.  He is also a master level social worker and has close to 20 years of experience in child welfare, providing direct behavioral health treatment and leading large behavioral health organizations. Rodrigo has previously served as National Clinical Care Director and State Director of Clinical Redesign's home and community based care program for Arizona and Nevada.  Before joining UnitedHealth Group, Rodrigo served as Chief of Operations and Clinical Officer for the Crisis Response Network (CRN) in Tempe, AZ in addition to a number of leadership positions with La Frontera of New Mexico and Arizona. Rodrigo has previously served on several boards/committees at the local, statewide, and national levels and currently serves on the Children’s Action Alliance board of directors.  Rodrigo holds a M.S. in Social Work with major concentration in family focused practice with Latino individuals and families from the University of Texas Pan American in Edinburg, TX.

Lisa Sessions, LCSW
Behavioral Health Clinician, UnitedHealthcare Community & State, Clinical Redesign

Lisa is a Licensed Clinical Social Worker with a diverse background in both clinical and process improvement roles. The majority of her experience is working with adults and elders in healthcare and community mental health. Lisa’s clinical experience includes CBT, DBT, and other Mindfulness based interventions. She has facilitated Seeking Safety groups and DBT groups with female veterans who survived military sexual trauma. Lisa spent seven years with the Department of Veterans Affairs as an LCSW working in their Homeless Programs and an In-Patient Domiciliary Program. She received the Six Sigma Green-Belt Certification with the VA and was detailed as a Management Analyst at the national level. Lisa was previously faculty at the University of West Florida and The University of North Florida as Visiting Assistant Professor, teaching social work students. Lisa is an active member on the Board of Directors for The Women’s Center Jacksonville, the rape crisis center for Duval, Baker, and Nassau counties.  When she isn’t working, you will find her in a yoga class, running, or riding her bicycle. 

Kathy Stillo, MBA
President & COO of Clinical Redesign, UnitedHealthcare Community & State Clinical Redesign

Kathleen Stillo is the President and COO of Clinical Redesign. She leads Finance, Strategy, Clinical and Housing Operations, IT/EHR, Post-Launch Markets, Training & Replication, Legal, and Marketing Communications. Her team is focused on implementing innovative care delivery models which incorporate social determinants of health, for better care at lower cost for our most vulnerable populations. Prior to joining UnitedHealthcare, Kathy co-led the Adult Health and Urban Health Institutes at Cooper Hospital in Camden, NJ. Kathy previously led strategic consulting teams for Bristol-Myers Squibb and Reuters. While in Camden, she was a member of the Board of Directors of the Camden Coalition of Healthcare Providers. Kathy earned her B.A. from Bowdoin College and M.B.A from Columbia Business School.

Irena Stjepic
Senior Care Advocate, UnitedHealthcare Community & State, Clinical Redesign

Irena Stjepic is a Licensed Clinical Social Worker with a passion for helping others.  She is a Behavioral Health Clinician with the Clinical Redesign team. Irena has over 12 years of healthcare and behavioral health experience serving individuals with complex behavioral and social needs.  She has participated in multiple diverse roles within the behavioral health field, including inpatient psychiatric treatment, individual therapy, group therapy, and evaluating individual’s psychiatric needs for appropriate level of care.  She also has experience with outpatient psychiatric treatment, case management, utilization management, supervisory  and private practice experience.  Throughout the years, she has helped grow a psycho-social program for adults with serious and persistent mental illness by implementing policies and program changes using a patient centered model. Irena notes that her greatest reward as a therapist is maintaining a positive approach throughout the therapeutic process, valuing the positive reinforcement with clients’ capacities and aspirations. She meets her patients where they are and guides them along their life journey helping them to focus on proactive steps towards a brighter future. Irena is based in Nashville, Tennessee, she enjoys reading, traveling, hiking, CrossFit and hot yoga.

Tom Tracy, MSW
Vice President, Strategic Business Planning, UnitedHealthcare Community & State, Clinical Redesign

Tom has spent his career serving vulnerable Medicaid populations with a focus on individuals with serious mental illness.  Tom has had the privilege to oversee a wide range of mental health programs including partial care, outpatient, ACT, in-home therapy, supportive housing, and juvenile justice involved, to name a few.  Prior to joining United, Tom served as the Chief Administrative/Strategy Officer for one of the largest community-based behavioral health organizations in Southern New Jersey.  Tom has trained at the local, state and national levels on topics such as care planning, service delivery, clinical documentation, outcome measures, gender, sexuality, diversity, suicide prevention and stages of change.  He embraces the philosophy that all people can experience recovery.  He enjoys growing talent and teaching to expand clinical skill sets.  Tom has a Masters in Social Work and a Master’s of Science in Management & Leadership.  He is a NJ Licensed Clinical Social Worker, an ACS and NJ approved clinical supervisor, and a Project Management Professional (PMP).  He and his husband are proud fathers to an adorable daughter.

Kate Warren, MSW
National Housing Operations Advocate, UnitedHealthcare Community & State, Clinical Redesign

Kate’s career and personal experience has led her to work with our complex care members via the Clinical Redesign's housing program.  Kate started her career as a project manager leading technology projects and process operations for large corporations. However, when her passion called her to do more to help the most vulnerable populations she took several years to volunteer for groups that worked directly with our community. Currently, Kate integrates her project management skills and passion for people to develop operation standards for the housing program of the Clinical Redesign Team.  Kate holds Masters Degrees in Geography and Pastoral Ministry. She has recently become a Court Appointed Special Advocate (CASA) volunteer, advocating for children in foster care.  

Terri Weisser
Instructional Design Consultant, Learning, Replication and Dissemination, Clinical Redesign Team

Terri brings over 25 years’ experience in learning and development across multiple industries. She has earned a coveted Brandon Hall Award for Best Use of Blended Learning and is a recipient of the UnitedHealthcare Service Hero award. Most recently with UHC’s Community and State, Terri drives to bring innovation and engagement to all facets of learning and works to help audiences interact with the content. Prior to joining Clinical Redesign, Terri worked with the Benefit Operations team where she led the creation of ground-breaking new hire program rooted in the corporate culture. She is also a UHG Culture Ambassador and enjoys mentoring others interested in instructional design and adult learning theory. Terri is based outside of Dallas with her husband, son, and rescue dog.

Lynn Whitaker, RNBC, CCM
Clinical Manager Northeast Region, UnitedHealthcare Community & State, Clinical Redesign

Lynn Whitaker has been a registered nurse for over 30 years, the majority of which has been in the psychiatric field. She obtained her Psychiatric & Mental Health Nursing Certification in 1992 and her Complex Case Management certification in 2016. Lynn has experience working in various settings including inpatient with adolescents & forensic units in the state institutions both in Ohio and Tennessee. She worked in Psychiatric Home Health for 8 years. She joined UHC over 10 years ago as a Behavioral Health Field & Utilization Care Manager and has worked as the Lead Transitional Care Manger until she accepted a position with Clinical Redesign in August 2018 as the Clinical Manager for the Northeast Team. Her vision includes giving a voice to those who don’t have one and ensuring that those we serve obtain the highest quality of care from the most compassionate clinicians.

Robin Wolfe, MSW
Behavioral Health Clinician, UnitedHealthcare Community & State, Clinical Redesign

Robin Wolfe is a Senior Psychological Examiner in the State of Tennessee who is currently a Behavioral Health Clinician with the Tennessee Clinical Care Team.  She brings over thirty years of mental health experience, with specialized training in psychological and neuropsychological assessment.  She has had the opportunity to work with a wide range of populations from infants to geriatrics, inpatient and outpatient psychiatric clients, and those with emotional, developmental, neurological and behavioral needs.  Prior to coming to UHC, she worked in a psychology private practice for fifteen years, and then as a health coach for seven years, where she developed a stress management program used by a global Fortune 500 company. Robin has a Bachelor’s degree in Biology and Psychology and a Master’s degree in Psychology from East Tennessee State University in Johnson City, Tennessee.  Living in one of the most beautiful areas in the county, she enjoys camping, hiking and exploring the region, as well as traveling and learning about the cultures of other countries.
