This activity is designed to meet the educational needs of physicians, physician assistants, nurses, and case managers who have an interest in personalized health-management support and services that include wellness programs, decision support, disease management conditions, complex-medical-condition support, case and utilization management programs, and physical health programs.

Activity Description: 

This educational activity consists of recorded presentations from 20th Annual National Conference held in Minneapolis, Minnesota on October 4-6, 2011.
A key part of the health overhaul law encourages the development of “accountable care organizations” (ACOs) that would allow doctors to team up with each other and hospitals in new ways to provide medical services. How does the ACO concept mesh with the medical home concept that facilitates partnerships among individual patients and their personal providers, and when appropriate, the patient’s family.

Learning Objectives
At the end of this educational activity, participants should be able to

  1. State the role of accountable care organizations in light of Health Care Reform.
  2. Identify how this concept may improve the quality of patient care and help lower health care costs and compare to the concept of the medical home.
