This activity is designed to meet the educational needs of physicians, nurses, and case managers who care for patients and/or manage their benefits and who have an interest in solid organ and blood/marrow transplant medicine.

Activity Description: 

This educational activity consists of recorded presentations from Innovations in Solid Organ and Blood/Marrow Transplantation held in Scottsdale, Arizona on March 7-9, 2011.
Medical and surgical breakthroughs over the past few decades have dramatically improved outcomes for children with CHD. When these patients reach adulthood, they can benefit from transitioning into an adult program that’s designed to meet their evolving needs. This presentation will discuss the challenges of this transition and the importance of providing uninterrupted, coordinated care between pediatric and adult providers, as well as outline management strategies to assist in this transition.

Learning Objectives
At the end of this educational activity, participants should be able to

  1. Review the importance of providing uninterrupted coordinated care between pediatric and adult providers for the CHD patient
  2. Outline management strategies to assist both the patient and the provider in transitioning care
