Activity description

End of life (EOL) is a frequent occurrence in the long-term care (LTC) setting as approximately one-third of nursing home residents die within one year of admission. Unfortunately, despite the frequency of death in LTC, there has been less focus on the EOL experience than in other sites of death.

This activity will focus on understanding what is important to individuals in LTC at the EOL such as pain and symptom management, alleviating burden placed on loved ones, spirituality, being treated as a whole person and avoiding prolongation of the dying process. The important need to discuss critical issues related to developing plans of care for these individuals will also be addressed.

Target Audience

This activity is designed to meet the educational needs of case managers, counselors, nurses, nursing home administrators, pharmacists, pharmacy technicians, physicians, psychologists, social workers (all practice types), marriage and family therapists and other health care professionals (HCPs) who provide care for individuals near the EOL.

Learning Objectives

  • Recognize the main themes that are important to individuals at EOL in LTC facilities.
  • State ways in which the EOL experience can be improved for individuals in LTC.
  • Identify ways in which dying residents and their families can be best supported in LTC facilities.
  • List barriers to EOL care and strategies to overcome these barriers.

About the Presenter
Christopher Jones, MD, MBA, attended the University of Scranton where he worked in a nursing home as a Certified Nursing Assistant during nights, weekends and summers. He attended medical school in Philadelphia as an aspiring geriatrician. Dr. Jones completed a residency in general internal medicine at Brown University/Rhode Island Hospital and fellowships in both geriatric medicine and hospice and palliative medicine at Duke University.

Professionally, Dr. Jones has focused on palliative and hospice care, working with adults of all ages with serious illnesses, especially at end of life. He serves as a consultant for hospice and palliative care organizations nationally, specializing in provider billing and coding, EPIC templates and launch and hospice level of care and eligibility determinations. In 2015, he was awarded the designation Fellow of the American Academy of Hospice and Palliative Medicine and chosen as one of Hospice and Palliative Medicine's Inspirational Leaders Under 40. He completed his Master of Business Administration degree at East Carolina University in 2017.

Dr. Jones currently serves on the faculty of the Perelman School of Medicine at the University of Pennsylvania and is the Co-Director of the Hospital of the University of Pennsylvania's Palliative Care Inpatient Consultation Service. His academic pursuits center on making the business of palliative care accessible to clinicians through writing and personally delivered education sessions as well as training medical and advance practice providers who will care for patients in the future.
