Autism Spectrum Disorder Part VI: Transition to Adult Care

Children with ASD will find themselves changing and growing as they enter into adolescence and adulthood. There are many factors that go into making the transition to adulthood and independent living as effective and successful as possible for a young adult with ASD. Long-term transition planning is an ongoing process that reflects the continuing development and changing needs of individuals with ASD.

This activity has been developed to provide an overview of transition goals and planning strategies for the changing needs of an adolescent with ASD and to emphasize the importance of early planning for these transitions. It will also highlight legal matters to consider for families and individuals with ASD as they transition to adulthood.

Learning objectives
At the end of this educational activity, participants should be able to:

  • Identify transition goals for the changing needs of an adolescent with ASD to include developing independent living competencies that incorporate personal care, household chores, financial skills and health care needs.
  • Explore transition strategies that can be taught in early childhood and explain the importance of early planning for transitions.
  • Discuss transitions from primary to high school and beyond for individuals with ASD.
  • Describe legal matters to consider for families and individuals with ASD.

About the presenter
Dr. Tisa Johnson-Hooper is a pediatrician in Detroit, Michigan and is affiliated with Henry Ford Hospital. She received her medical degree from the Mayo Clinic School of Medicine and has been in practice for more than 20 years. Her practice offers a family-centered medical home approach to treating children with special health care needs. Dr. Johnson-Hooper emphasizes the value of family partners and strives to make her services convenient, family-friendly and easy to use. She regularly partners with family navigators and liaisons, and much of her practice has been built around and guided by family feedback.
