Early recognition of a change in pulmonary status improves the clinical management of residents of long-term care facilities. Health care personnel are frequently responsible for assessing the respiratory status of residents and identifying early signs and symptoms of a change in respiratory condition. This is essential to providing early intervention, but there are challenges in differentiating between onset of an acute illness and an exacerbation of a chronic condition. Optimal care requires skills and training that include education on accurately assessing physical findings, relevant patient history, vital signs and available diagnostics. This activity will discuss the etiology of respiratory illness and highlight assessment skills and interventions. Details on pneumonia and chronic obstructive pulmonary disease (COPD) will also be described.

Post-acute discharge planning that is adequate and specific to the patient’s care needs is essential to ensure continuity of care, reduce the rate of unplanned re-admissions and complications and improve the patient’s quality of life; appropriate planning also reduces inaccuracies and oversights during transitions of care. This activity aims to increase competency surrounding the post-acute discharge planning process. It will discuss the various levels of care and types of services available for post-acute discharge planning and examine the eligibility requirements specific to each setting, including how payer criteria can impact planning.

Opioid prescribing guidelines provide clear and comprehensive, evidence-based recommendations incorporating current information and practices for practitioners. This activity will examine the scope, impact, key statistics and factors contributing to the opioid crisis and highlight the current recommended opioid prescribing guidelines, their challenges and the role of state prescription drug monitoring programs (PDMP). Strategies to aid in implementing the opioid prescribing guidelines and to mitigate the opioid crisis will also be discussed.
