Intended Audience
This activity is designed to meet the educational needs of physicians, physician assistants, and nurses who have an interest in personalized health-management support and services that include wellness programs, decision support, disease management conditions, complex-medical-condition support, case and utilization management programs, and physical health programs.

Activity Description
This educational activity consists of recorded presentations from 20th Annual National Conference held in Minneapolis, Minnesota on October 4-6, 2011.
This activity will focus on innovations and trends in solid organ and blood/marrow transplantation, oncology, kidney disease, heart disease, physical health, disease management, consumerism, and health and well-being.

Learning Objectives
At the end of this educational activity, participants should be able to

  1. Compare theoretical and practical ideologies regarding the characteristics of our health care system that contribute to the soaring cost of health care.
  2. Recognize and apply emerging trends and innovations in the prevention and management of complex medical conditions in order to avoid or postpone the need for transplantation, including VAD therapy for end-stage heart failure, and the medical management of chronic kidney disease.
  3. Define the concept of accountable care, and identify how this concept may improve the quality of patient care and help lower health care costs.
  4. Discuss clinical management measures for pediatric patients, including those with congenital heart disease, children who survive intensive care, and children with obesity-related disease.
  5. Name the most pressing issues facing the health care system in light of the current status of health care reform.
