Activity Description
This activity will be a 'Train the Trainer' Program with designated site leadership staff (NP/PA Champion, Clinical Advisor, Clinical Service Manager, Medical Director). The purpose of the program will be to provide a comprehensive review of Advanced Care Planning to enable the identified site champions to implement the program (with tools) in the individual markets.

Learning Objectives
At the end of this educational activity, participants should be able to

1. Discuss the elements of a patient/family care plan meeting and end-of-life communication
