Developing a Clinical Initiative on Chronic Kidney Disease in a Risk Adjustment Model

Activity Description
The goal of this presentation is to provide information on the healthcare delivery initiatives of the Center for Medicare/Medicaid Services (CMS) and the IPA's plan to develop a clinical initiative for chronic kidney disease.

Learning Objectives
At the end of this educational activity, participants should be able to

  1. Understand the History of Medicare and Current Status of HealthCare Delivery for Medicare Advantage Patients, as determined by federal statutes
  2. Explore the prevalence, incidence, documentation, and management of all stages (early and late) of chronic kidney disease, based on current evidence-based medicine (peer-review literature) and best practice models developed by the National Kidney Foundation
